To request an RMA please do so by going to or Ticket System at www.siliconeelectronics.com/ticketsystem
and create a New Ticket so you can receive a Ticket number. Once you have your Ticket number you will
need to download and print an RMA Form which can be located to the left of this page. If you do not have
a printer then please fill out the form below and include with your packaging a piece of paper with your
Ticket Number written down on it.


eBay customers need not to apply for a Ticket number you will be e-mailed one. But, please, either fill out
our Online RMA Application Form below or print one (you can do this by choosing one of the following RMA Form
formats to the left of the page), fill it out and include it with the packaging. All RMA customers who request
an RMA can view the progress of the RMA in our Ticket System! eBay customers will be e-mailed their Ticket
Number. Everyones login with our Ticket System will be their e-mail and their Ticket Number.
